* Please confirm you available to attend board elections on August 8th from 5:00pm-6:30pm.

By running for board you agree to attend at least 75% of all board meetings and events

* Please confirm you available to attend board retreat on August 21st from 10:00am-5:00pm

* TEEN: I have read the job description for the board position and the additional responsibilities of the position for which I am running. I understand the job requirements and, if elected, I the required amount of time and energy necessary to fulfill my responsibilities.

* PARENT: I have discussed the obligations and job descriptions with my child and I give them permission to run to run for the office chosen above. I also will support them in getting to and from meetings and events.
